Introduction. The analysis of prescriptions in community pharmacies confirms that patients often rely on the advice of several physicians to confirm the diagnosis and the ordered treatment. Based on the collected data, it could be pointed out the range of physicians specializations who generally administered drugs for patients and determine which diseases predominate especially in elderly patients, which should contribute to a certain orientation of implemented pharmaceutical care (PC). Aim. Evaluation of the range of life-style diseases and the physicians’ specialization who administer the drugs to patients included in the PC process in a community pharmacy. Matherial and methods. The study was conducted between January 2014 and June 2015 in Jarocin within 54 patients (41.0 % men and 59.0 % women) at age over 65 years who were counselled by family doctor and at least two other professionals. Results. Collected data confirmed that 75.9% of patients were treated for hypertension, 33.3% had hypercholesterolemia and 24.1% suffer from type 2 diabetes. The study indicates that 51.8% of patients were consulted by two physicians and, in extreme cases (3.7%) the treatment was carried out by up to five different specialists. A detailed analysis indicated that 66.7% of patients were treated by a cardiologist, 55.6% by the ophthalmologist and 25.9% by the endocrinologist. As a result of the pharmacotherapy 66.0% of patients took more than 8 preparations. The family doctor prescribed in general between 1 and 3 formulas within 44.4% of patients. It is also disturbing that 51.8% of patients were involved in casual self-treatment using between 1 and 3 preparations. Conclusions. Identification the group of physicians’ specialists, who counsel the elderly patients most often, and the indication of drug problems and its consequences, may have a beneficial influence on the pharmacoeconomic effect resulting from PC process.
